Medical Case Management

The medical social work in case management program provides a variety of services.  Services include but not limited to:

  • Brief short-term mental health therapy (up to 5 sessions.)
  • Assisting clients procuring and obtaining services from various agencies
  • Helping patient's apply for Social Security disability benefits
  • Applying for MIBRIDGES benefits
  • Enrolling for healthcare coverage through the insurance marketplace
  • Psychoeducation and education materials.
  • Assisting veterans and receiving veterans services
  • Obtaining durable medical equipment
  • Assisting patients and family members of patients who have dementia obtain resources
  • Assistance with long-term care placement when needed

We cater to all members of the Sault Sainte Marie tribe of Chippewa Indians are served.  Members of any federally recognized tribe and their families are also eligible to receive these services.
 
The patient's health care provider to make a direct referral or the patient can call 906-632-5200 extension 22620 to schedule an appointment.

The other health centers in the Sault tribe health division do not have a medical social work and case management program at this time.  However if services are needed in outlying areas arrangements will be made to assist patient's requiring these programs services living in outlying areas within the 7 County service area.  However any effort will be made to assist Sault tribe members with information and resources regardless of where they live.
