Medical Case Management

The medical social work case management program provides a variety of services.  Services include but are not limited to:

  • Brief short-term mental health therapy (up to 5 sessions.)
  • Assisting clients in procuring and obtaining services from various agencies
  • Helping patients apply for Social Security disability benefits
  • Applying for MIBRIDGES benefits
  • Enrolling for healthcare coverage through the insurance marketplace
  • Psychoeducation and education materials.
  • Assisting veterans and receiving veterans’ services
  • Obtaining durable medical equipment
  • Assisting patients and family members of patients who have dementia to obtain resources
  • Assistance with long-term care placement when needed

We offer these services to all Sault Tribe of Chippewa Indians members and their families, as well as tribal members of any federally-recognized tribe and their families. 
 
The patient’s primary care provider can make a direct referral, or the patient can call 906-632-0109 or extension 42620 to make an appointment. 

The Sault Tribe Health Center is the only tribal health center that has a Medical Social Worker; however, if a patient in an outlying area within the Sault Tribe seven-county service area needs these services, arrangements can be made to assist them.
