Sault Tribe Health Physical Activity Referral Program FAQ 

Who is eligible for the Sault Tribe PA Referral Program?

All Sault Tribe members who have seen a primary care provider at a Sault Tribe Health Center within the past 6 months.  All referrals must be made by a Sault Tribe medical provider.  If you have not seen a Sault Tribe Primary Care Provider within the past 6 months, please call scheduling to make an appointment with your primary care provider.

What services are still being offered through the Sault Tribe PA Referral Program?

All patients in the Physical Activity Referral Program will receive direct 1-on-1 with an Exercise Specialist 1x/wk for 6
months. Patients will complete an initial consultation, fitness assessments, and receive an individualized exercise prescription.

Can I still receive Nicotine Dependence Program services at this time?  How do I contact someone to complete the initial appointment?
